Flow from reception to delivery

World Tournament Participating Store List

1. Prescription reception

We will keep your "prescription" and "medicine notebook" issued by the hospital. At the time of reception, we will check if there are any changes in your physical condition, if there are any remaining medicines, and if you wish to use generic drugs.

In addition, we ask patients who visit our office for the first time to fill out a confirmation sheet regarding their constitution and side effects.

About pharmacy waiting times

If you use the prescription sending function of our recommended medicine notebook app, you can shorten the time you have to wait at the pharmacy, and you can also spend time outside the pharmacy until the medicine is ready, so you can use the waiting time. I can do it. Please check with a store employee for details.

2. Prescription inspection

We check whether the prescribed medicine is appropriate for the patient by comparing it with the medication history (medicine record) and the information in the medicine notebook. If there are any unclear points, we will check with the prescribing doctor (question inquiry) as necessary.

Inquiries to hospitals (question inquiries)

There are various types of inquiries made at pharmacies. At our company, we disclose some cases (pre-avoid cases) in which side effects, interactions, insufficient therapeutic effects, etc. that are disadvantageous to patients have been avoided or reduced after confirmation by pharmacists.

4. Comprehensive inspection

In order to ensure that all medicines are prepared correctly and that patients can use them with peace of mind, we perform a comprehensive check, including printed materials that are handed over.

5. Medication explanation/Medication delivery

While listening to the patient's symptoms, we will explain the efficacy, effect, usage and dosage of the medicine in an easy-to-understand manner.

In addition, based on the information obtained from the patient, we will think together about the patient's concerns about medicine. If you have any questions or concerns, please feel free to contact us.

About home care

At pharmacies, in addition to cases in which patients bring prescriptions directly to pharmacies, it is difficult for patients to visit hospitals or pharmacies directly, and pharmacists visit patients who are being treated at home. We provide “home medical care”, a service that delivers medicines.

6. Medication history

In the "medicine history", we record the content of the medicines the patient is taking, the side effects and allergy history that the patient has visited, and the explanations given by the pharmacist each time.

"Medicine history" is used as a reference for sharing information with medical institutions, in addition to the patient's treatment progress and future drug explanations.

7. Follow-up during medication period

During the period of taking the medicine, in addition to confirming whether the medicine is being taken properly and whether there are any side effects, we will ask about any problems you have when taking the medicine, changes in your physical condition, illness, lifestyle habits, etc. to continuously support health management.

Pharmacy introduction

The pharmacy of choice for patients in the region

N4 pharmacy

The Tsuruha Group continues to challenge new concepts with the aim of becoming the pharmacy of choice for patients in the region.

"N4 Pharmacy", located along Kita 4-jo Teine Street in Sapporo, Hokkaido, is a new concept design that the Tsuruha Group is aiming for, and will serve as a model pharmacy that will represent the Tsuruha Group in the future.

New concept waiting room

The waiting room is coordinated with warm wood tones and has a bright and open design. On the other hand, the medicine delivery port has a partition to protect privacy. In addition, as a pharmacy that supports healthy life expectancy, we have set up a “health self-check” space and have created an environment where body composition, blood pressure, etc. can be measured.

The Tsuruha Group has introduced a variety of systems to ensure safe, secure, and efficient preparation of medicines. Patients' medication histories (medicine records) can be managed on a computer with the latest information, allowing immediate response to inquiries. In addition, as a "mistake prevention system for picking", we scan the bar code of the medicine with a small terminal and check it by human eye and machine. In addition, we have introduced a machine that assists in the preparation of medicines, making it more efficient so that we can have time to talk with patients.

3. Accompany the doctor

A doctor actually examines each patient. While understanding the content of the patient's talk, if the doctor or nurse has any questions about medicine, I will make a prescription proposal.

4. Confirm the contents of the prescription

We will reconfirm the contents of the medicine based on the prescription issued by the doctor and the contents at the time of rounds. In addition, if there is a request for change or addition of medicine from the facility, we will contact the doctor and request a change of medicine.

5. Prepare medicine

To make it easier to manage medicines, tablets and powders to be taken at one time are put together in a single package. We dispense medicines according to the wishes of patients and caregivers, such as packaging medicines that are adjusted and taken, such as regular medicines, in separate packages. In some cases, by separating the colors for morning, afternoon, and evening, we devised ways to prevent mistakes in drinking from the letters and colors.
